Subject: Datafeed to multiple terminals for single trader

A few years ago there was a form that could be filled out which detailed the usage of data by operator/trader, allowing for traders who used multiple data platforms to avoid paying multiple fees. One trader Reuters and Bloomberg on his desk would pay only one licence for real time NYSE data, even though the data was being supplied to both terminals.
Does this form still exist and where could I find it?

I would like the same form for my company.

Also, I would like to have a phone number so I could actually talk to someone about this. I can't find a phone number on my bill or on this web site.
